DENTAL INSURANCE
Our office is happy to assist with families who are covered by dental/orthodontic insurance. Please read your policy to be sure you are fully aware of any limitations of the benefits provided by your policy.
Please bring a copy of your insurance card with you to your initial visit. Make sure you have the group number and phone number to reach your company. We will contact your company and help determine the exact benefits your company has made available for you. For your convenience, we will file your orthodontic insurance claims for you at no charge.
Please provide our office with any change in your insurance coverage during treatment to prevent reassignment of any insurance portion to your personal account. Remember, you are ultimately responsible for your insurance.
PAYMENT OPTIONS
OPTION #1-FULL PAYMENT AT START OF TREATMENT
There is a 5% discount from the total treatment Fee if payment is received in full at the beginning of treatment. (This discount cannot be extended towards credit card payments).
OPTION #2-IN-HOUSE FINANCING
A credit report may be required for this option. A payment plan will be determined by your credit score. This payment plan will consist of a down payment and monthly payments. With this option, payments are due at the beginning of each month to avoid any late fees.
OPTION #3-CAPITAL ONE HEALTH CARE FINANCE
This independent financing company has offers no down payment and spreads payments over 18-60 months. This independent financing company offers a low subsidized interest rate for those who qualify.
